What is your cancellation policy?
If you need to cancel your appointment, please give us at least 24 hours notice. If you don’t give us 24 hours notice, you may be billed for the entire cost of the session. It’s important to note that insurance companies do not provide reimbursement for canceled appointments.

Is counseling confidential?
Your trust in us is essential and we treat confidentiality seriously. What happens in therapy sessions is private, except in very specific, extremely limited circumstances. We use state of the art technology to encrypt your electronic records and keep paper records under lock and key. We can not discuss your treatment with anyone or release any information about you without your written permission, except in the following circumstances:
- Suspected past or present abuse or neglect of children, adults, and elders must be reported to the proper authorities, including the Department of Human Services and/or law enforcement.
- If we believe that you are seriously in danger or harming yourself, or you have threatened to harm another person, we must contact the proper authorities and make a reasonable effort to contact family members and anyone who might be in danger.
- If you use your health insurance to pay for services, the health insurance provider and other third-party payers have the right to review your records.
- When a court orders the disclosure in a civil, criminal, or administrative proceeding or as otherwise required by state or federal laws, we are required to release your records.

If you would like us to share your records with another healthcare provider, please sign our Release of Information form and fax or mail it to Prairie Wellness.
Where are you located?
Prairie Wellness is located at 3209 Ingersoll, Suite 100 in Des Moines. There is plenty of well lit parking on the east side of the building. We are also on the Des Moines DART bus line. Do you take credit cards?
We accept cash, checks, and most major credit cards for all of the services we offer at Prairie Wellness. Payment is expected at the beginning of each counseling session.
Do you accept insurance?
Psychotherapy sessions may be covered, in full or in part, by your health insurance plan. Please call your health insurance provider to determine what coverage you have, what your deductible and co-pay may be, and whether we are in-network or out-of-network (check under the name of the therapist you’d like to see, not Prairie Wellness). Please note that you are ultimately responsible for paying for services. If your insurance company denies payment, you will be charged.
If we are out-of-network, we will be happy to provide you with a receipt to file your claim. It will be up to your insurance company to determine whether or not you receive reimbursement for your fee.
Insurance does not cover our Mindfulness-Based Stress Reduction class.

What do I need to bring to my first psychotherapy appointment?
If you are using health insurance, please bring your insurance card. All clients should bring identification and a credit or debit card. Please download new client forms from our website here, complete them, and bring them with you. If you aren’t able to download the forms, please make arrangements with your therapist to come 15 minutes early to fill them out at our office.
